Efficacy of Large Loop Excision of the Transformation Zone(LLETZ) for Cervical Intraepithelial Neoplasia(CIN)

Abstract

|
This study evaluated the efficacy of LLETZ in the treatment of CIN. Eighty four patients with abnormal cytologic smear were studied. In all patients, coposcopically-directed punch biopsy was performed. And the patients received LLETZ as an
outpatient
procedure. This procedure was well-tolerated, and only minor bleeding occurred in a few patients. There were 4 cases(4.8%) of microinvasive cervical carcinoma that were missed by coposcopically-directed punch biopsy, the diagnosis being made
after
the
patients underwent LLETZ. There was a large disparity in the histological analysis between colposcopically-directed punch biopsy and LLETZ specimen. This disparity would not have been detected if the patients were treated by ablative methods.
Therefore
LLETZ is preferred for the treatment of the patients with CIN, especially CIN grade III, because the whole excised lesion can be examined histologically and diagnostic error is reduced. LLETZ seems to combine the benifits of the gold standared
cone
biopsy with that of local ablative methods.
